Service Description:
The Dementia Support Manager and Dementia Support Worker are dedicated members of staff supporting people living with dementia and their carers, by helping to maintain independence, improve a sense of well-being and enabling people to have control of their lives.
This service can be carried out at home, at their offices or other locations and can be face to face, by phone, letter or email. They offer emotional and practical support, guidance and information on dementia, and caring for a person with dementia from diagnosis or before through to end of life. They offer assistance with applications for welfare benefits, provide information on services within the borough and make referrals to statutory and voluntary agencies.
Carers’ education courses are also provided on Dementia Care and related topics. The Dementia Support Team provides support and information to people newly diagnosed and their families at Groveland’s Memory Clinic on alternate Friday mornings.
Days & Hours of Opening: Monday to Friday, 9.30am – 5pm
